Our multidisciplinary team provides supervised and monitored exercise, education and support for individuals with new-onset and chronic heart and lung conditions.<\/strong><\/div>\n

The primary goal of cardiac or pulmonary rehabilitation is to enable participants to achieve optimal physical, psychological and social functioning through exercise training and lifestyle change.<\/p>\n

Our cardiopulmonary rehabilitation team provides individualized and personalized supervised and monitored exercise, chronic disease and risk factor education, nutrition counseling and mental well-being assessment and counseling for persons with heart or lung diseases.<\/p>\n

Our multidisciplinary team is made up of exercise physiologists, registered nurses, registered dietitians and licensed social workers who coordinate care with cardiologists, pulmonologists and primary care physicians.
